A clear explanation of your rights responsibilities and choices before starting services with bttr. Informed consent ensures you understand our supports how they work and your freedom to ask questions or withdraw at any time.

What is informed consent

Informed consent means the participant or authorized representative understands who bttr is what services are offered how they are funded how participation works how personal information is protected and agrees voluntarily to begin supports. Consent must be free of pressure confusion or fear. Participants may ask questions request accommodations or withdraw consent at any time.

Who may provide consent

  • The participant if legally able
  • A parent or guardian if the participant is a minor
  • A conservator or legally authorized representative
  • A designated support person with legal authority

Participants may include family advocates or Regional Center staff when reviewing or discussing consent.

Services covered

  • Independent Living Services ILS. One on one coaching in skills like budgeting cooking communication health and advocacy.
  • Supported Living Services SLS. In home support focusing on routines safety housing and autonomy including 24 hour support when authorized.
  • Community Integration Training Program CITP. Small group coaching focused on social connection job readiness public safety and independence.

Services are delivered under an approved IPP ISP and POS through Regional Center or the Self Determination Program with no out of pocket costs to participants.

Requirements to begin services

  • Autorización del Centro Regional o SDP
  • Evaluación de entrada completada
  • Reviewed and agreed upon initial ISP
  • Residence within an approved service area
  • Willingness to participate respectfully in services

Withdrawing or ending services

Participants may stop services at any time. We request thirty days written notice to support a smooth transition. Services may end if consent is withdrawn authorization expires relocation occurs outside the service area goals are completed non participation continues or a serious safety or conduct issue arises. All transitions are handled respectfully and collaboratively.

Notice of withdrawal

To end services participants may notify:

  • bttr by emailing info@bttrliving.com or notifying a coach
  • The Regional Center Service Coordinator or FMS provider under SDP

Please submit notice at least thirty days in advance unless there is an emergency or earlier agreement. For serious safety violations bttr may end services immediately with written notice to all parties.

Consent to communication

By consenting to receive services participants allow bttr to communicate by phone email text and in person coordinate with Regional Center staff maintain secure records share required documentation and request updated consent if services change. bttr never sells shares or uses participant information outside authorized service delivery.
